Clients must be in agreement with our billing policy, in order to set up an account and prior to confirming an appointment. After submitting this form, please call our office at (434) 951-9225 to provide your credit card information. Once we have billed you for our services, you can expect an invoice to be sent to the email you provide and your credit card on file will be processed shortly afterwards. To update your existing account, please include your account number at the top of the form.
